About GUIDE
The GUIDE (Guiding an Improved Dementia Experience) program is a new initiative from the Centers for Medicare & Medicaid Services (CMS) designed to improve quality of life for people living with dementia - and their caregivers.
As a participating primary care provider, Ennoble At Home is honored to bring these services directly to your home through our mobile house calls program.

Patients living with dementia who are covered by Medicare and meet program criteria are typically eligible. Our team will confirm eligibility during your initial conversation.

The GUIDE program is part of your loved one’s Medicare coverage — there is no additional cost for participation.

If you are already under the care of an Ennoble Care provider, you will not need to change doctors. However, in order to participate in Ennoble At Home's GUIDE services, you will need to be a patient of an Ennoble Care provider.

Ennoble at Home currently offers GUIDE enrollment in Georgia, Kansas, Maryland, New Jersey, New York, Oklahoma, Pennsylvania, Virginia, and Washington, D.C.
